Claude Code 多模型协作的 SESSION_ID 自动管理工具。通过 PreToolUse hooks /skill 在每次 MCP 调用前注入会话状态，防止跨任务上下文丢失和会话混乱
安装完毕后，只需重启 Claude Code 即可生效，无需手动配置 hook 或预先创建 sessions.json。但 hook 只负责读取会话状态，MCP 返回的 SESSION_ID 仍需手动更新或配合 session-sync skill 写回。
