Code review is a verification problem disguised as a language problem. A single LLM call against a diff produces fluent prose with no guarantee any of it is true - findings get invented, lines get cited that were never changed, and there is no mechanism that catches a hallucinated defect before it reaches the developer. Manual review avoids this failure mode but doesn't scale: reading, cross-referencing, and judging every changed line by hand is exactly the high-volume, well-defined cognitive labor automation should be doing.
