LLM 支持按 provider/model 按请求切换: app.agents.llm_router.get_llm(provider, model) 接受 provider + model 参数, 内部按 (provider, model) 作 cache key (lru_cache)。get_llm() 不带参数仍走环境变量里的默认组合, 向后兼容老调用点。
